Union Hospital Health Group FATHERHOOD COACH-GRANTS in TERRE HAUTE, Indiana

Position Summary: Key member of the perinatal navigation team specializing in connecting and engaging with fathers/male partners to promote healthy family involvement and relationships.

Key Duties and Responsibilities:

  • Assists fathers in their homes, community, or clinic setting. Communicates to clients/patients the purposes of the program and the impact it may have on their wellbeing.

  • Helps fathers identify socio-economic issues that affect their overall health and develop health/social management plans and goals.

  • Support peers in utilizing effective strategies to engage fathers in program services and community resources, including but not limited to, mental illness, substance abuse, housing, employment etc., that affect family well-being.

  • Assist with planning, preparation and providing parent groups, effectively delivering father focused groups and events.

  • Participate in community events, workgroups and committees to promote responsible fatherhood and the importance of fathers’ contribution to the safety, permanency and well-being of children.

  • Documents activities, service plans, and outcomes achieved by client in an effective manner.

  • Assists clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and /or social services.

  • Facilitates communication and coordinate services between providers and the clients/patients.

  • Establishes positive, supportive relationships with participants and provides feedback to other members of the team.

  • Works to reduce cultural and socio-economic barriers between clients and institutions.

Qualifications Requirements

  • Learn and understand Healthy Start program Goals, Objectives, and Priorities

  • Ability to conduct outreach to meet with program participants in non-clinical settings

  • High school diploma or GED; at least 3 years of experience directly related to the duties and responsibilities specified. (required)

  • Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.

  • Applicant must have a working vehicle to successfully facilitate transportation to/from but not limited to: home visits, community visits, clinic sites. (required)

  • Become Certified Community Health Worker within 6 months of starting position
