Centralized Intake Coordinator

Description

Responsible for the review and processing of skilled nursing referrals, inputting data into computer, following up with case managers and verifying benefits. Provides organization to the referral process through consistent intake information and effective communication in order to provide continuous quality for the customer.

  • Demonstrates efficient and effective management of the referral.

  • Processes all information specific to the referral and directs appropriately to the Clinical Liaisons, referral source, and Care Center.Evaluates appropriateness of referral.

  • Requests assistance as needed from clinical staff

  • Obtains authorization for skilled services from various payor sources.

  • Maintains knowledge of reimbursement for Medicare, Medicaid and other payor sources.

  • Communicates payor information to Care Center.

  • Provides information on the full scope of available services to patient, family, physician and referral source if needed

  • Contributes to team effort by accomplishing related tasks as assigned

Requirements

Must adhere to the Company’s Code of Conduct and Business Ethics policy including documentation and reporting responsibilities

  • Knowledge of medical terminology and Managed Care Plans Customer Service/Marketing experience strongly preferred

  • Must be capable of maintaining regular attendance and work evenings and weekends

  • Must be able to multitask

  • Must be well organized

  • Must demonstrate excellent verbal and written communication skills

  • Must be able to work in a fast paced environment

  • Must be able to maintain confidentiality regarding patient and company proprietary information

  • Must have the ability to relate professionally and positively and work cooperatively with other employees at all levels

  • Must possess computer skills of the types and at a level necessary to accomplish the job

Sedentary work involves lifting no more than 10 pounds at a time and occasionally lifting or carrying articles like docket files, ledgers, and small tools. Although a sedentary job is defined as one which involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties. Jobs are sedentary if walking and standing are required occasionally and other sedentary criteria are met.
