The League for The Blind and Disabled DeafLink Case Coordinator in Fort Wayne, Indiana

POSITION SUMMARY: Incumbent provides case coordination services for consumerswho are Deaf or have significant hearing loss on a fee-for-service basis, primarily in northeast Indiana.EXPECTED RESULTS: The efforts of this position should result in: 1.Provide services to the minimum number of active consumers identified in the DeafLink Measurement Frameworks and Report Form, 2. Minimum percentage goal identified in the DeafLink Measurement Frameworks and Report Form set by DeafLink active consumers will be achieved, 3. Minimum percentage goal identified in the DeafLink Measurement Frameworks and Report Form of consumers served will increase their ability to access transportation, housing, health care and/or adaptive equipment, 4. Minimum percentage goal identified in the DeafLink Measurement Frameworks and Report Form for active consumers will obtain the community services and supports they need, 5. Submit billing to receive the maximum amount of funding available through the State of Indiana case management contract.ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.Service DeliveryiAssist consumers in identifying goals that they would like to accomplish. Provide case coordination services for consumers in the DeafLink service area in accordance with identified consumer goals and/or written plans.Assist consumers in accessing needed community services, supports and resources.Teach consumers how to resolve problems or issues independently and advocate for their own needs. Monitor consumer progress relating to consumer goals and/or written plans.Provide work incentives and state and federal benefits trainingPublic Relations/MarketingInform consumers, businesses, government entities, and social service entities about the services provided through DeafLink and their benefits.i Maintain positive relationships with the deaf community, interpreters, co-workers and other professionals.Administrative and ClericalSubmit accurate and complete weekly time sheets that are consistent with information contained in case coordination forms. Develop reports and provide statistical data as requested by the League administrators.Provide policy and procedure recommendations relating to the DeafLink program, as needed. FinancialiiiiiiiiProvide the League Director of Finance and Operations and DeafLink Director with information needed to send billing invoices to case coordination funding sources monthly and as requested.Planning iiiiiiiiPlan and coordinate self-help, self-advocacy training sessions for consumers.PersonneliiiiiiiIncumbent is not directly responsible for any League personnel activities.SUPERVISORY RESPONSIBILITIES: Incumbent does not supervise any staff.QUALIFICATIONS, REQUIREMENTS and DEMANDS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. BA/BS in Health and Human Services; Psychology, Counseling, Bachelor of Social Work and/or Master of Social Work (M.S.W.) preferred. Extensive knowledge of the Deaf culture is required. Knowledge of, or ability to learn federal, state, and local civil rights provisions and support systems available to people with disabilities required. Prefer a qualified person with a disability.LANGUAGE SKILLS: Excellent receptive and expressive sign language skills in American Sign Language. Ability to speak effectively before groups of customers, DeafLink business customers, and community groups.
