Reid Health LPN in Connersville, Indiana

A “Licensed Practical Nurse” is a person who holds a valid Indiana license, and who functions at the direction of the Registered Nurse, Physician, Osteopathic Physician, Dentist, Chiropractor, Optometrist and Podiatrist in performance of activities requiring special knowledge or skill.

  • Appropriately recognizes and reports abnormal assessment data.

  • Collaborates with RN to create appropriate plan of care.

  • Effectively carries out plan of care.

  • Recognizes success of or modifies plan of care

  • At the direction of the RN, initiates nursing protocols and standing orders as condition dictates.

  • Appropriately communicates patient condition to the RN

  • In collaboration with and at the direction of the RN, provides care based on identified needs.

  • Demonstrates that she/he can work independently within scope of practice.

  • In collaboration with and at the direction of the RN, works well with the multidisciplinary team.

  • Uses appropriate resources – Primary RN, Charge Nurse, Unit Manager, Directors, Administrative Coordinator, Pharmacy, etc.

  • Demonstrates good time management by having no unjustified overtime.

  • Informs supervisor when hours worked may accrue overtime and incidental overtime.

  • Compliant with attendance policy.

  • Offers assistance to team to maintain unit productivity – asks “What may I do to help you leave on time?”

  • Collaborates with team and management to develop cost-effective solutions and improve workflow processes.

  • Uses team huddles and check-backs to proactively report changes.

Education: Must be a graduate of a Licensed Practical Nursing program.

Experience Required: At least one year experience as a Licensed Practical Nurse.

Experience Preferred:Previous experience in a residential or psychiatric facility preferred.

Licensure: Licensure in the State of Indiana as a Licensed Practical Nurse is required.

Certifications: Must maintain competence in BLS after initial training.

Must be at least 21 years of age.

Day shift. 36 hours Weekly, 6:00a-6:00p
